ID is required for all piercings:
18 and over, a physical ID that is not expired will need to be presented at the time of your service**if you are under 18, PLEASE review requirements....The Minors birth Certificate, NO EXCEPTIONS & the parent or guardian must be present and their ID/Name must match. If a guardian, please provide guardianship paperwork.
***All minors under 12, please have their immunization card, birth certificate and parent ID as well, Legal Birth Certificate is required as their ID until they are 18.
**if over 12 years old Current photo ID, for any minor over the age of 12(school ID,school portal, permit, passport, government issued ID)no immunization records required! Parent ID & Birth Certificate is needed.
